Advance Practice Provider (APP) Application
Thank you for your interest in applying for the Stanford and/or LPCH Advance Practice Provider staff.
Please note: when requesting privileges for both hospitals you need only fill out one application.
The APP Application process is divided into six sections. Please go to each step and follow the directions carefully.
PROCESSING TIME:
Our standard processing time is 90 – 120 days following receipt of a completed application.
